Media Waiver Acknowledgment of Risks and Waiver of Liability
I, the applicant named in this registration form, hereby acknowledge that I am voluntarily participating in the Utah Islanders training sessions and or program activities. I recognize that the training involves physical activity and inherent risks which may cause serious injury or even death. I affirm that I am in good health and physically capable of engaging in this activity.
In consideration of being permitted to participate in the training session, I agree to assume all risks and responsibilities surrounding my participation. I hereby release, waive, discharge, and covenant not to sue the Utah Islanders, its directors, officers, employees, volunteers, agents, and any other participants (collectively "Releases") from all liability to me, my personal representatives, assigns, heirs, and next of kin for any loss, damage, and any claim or demands on account of injury to my person or property or resulting in my death, whether caused by the negligence of the Releasees or otherwise because of my participation.
Photography and Video Release
I grant the Utah Islanders and its agents the right to photograph and/or videotape me and further utilize my name, face, likeness, voice, and appearance in connection with exhibitions, publicity, advertising, and promotional materials without reservation or limitation. I waive any right to inspect or approve the finished version(s), including written copy that may be created in connection therewith.
